Combination Request Form Instructions
Please read before submitting a request.
In order for a combination to be reissued or to place an order for safe keys, this form must be completed in its entirety and payment in the amount of $25.00 per combination or key must be received.
Upon completion of this form, you must attach either a valid proof of purchase (i.e. receipt, bill of sale) or a valid letter from a law enforcement agency identifying the safe by its serial number and indicating that the safe is in fact in your possession and has not been reported stolen.
Any documents outside of these described above are not acceptable and will cause your request to be delayed or denied.
Once you have completed this form and obtained the necessary Notarization, please fax or email this form along with the required proof of purchase or letter from a law enforcement agency to the fax number or email address provided below.
Once we have received the required documents, we will process your request and send the combination or keys for your safe via United States Postal Service to the shipping address you provide on this form or to the email address you provide.
Please allow 7-10 business days for all requests to be processed and shipped or emailed, if a combination is requested.
